Payment Policy

In order to keep charges at a minimum, it is necessary to ask for PAYMENT AT THE TIME OF SERVICE. Your cooperation will be greatly appreciated. Please remember that, even with insurance, you are ultimately responsible for the care you receive.

Insurance

Please be sure to bring your insurance card with you. Our office participates with Medicare and several managed care plans and preferred provider organizations (PPOs). We will file all charges and accept assignment of benefits for these plans. You will still be responsible to pay any deductible, co-payment, and non-covered charges at the time of service.

Referrals

If your insurance requires a referral to see one of our specialists, it is your responsibility to see that the referral is received by our office prior to your appointment. Please call our office to be sure the referral has been received. Because of insurance requirements, we will not see you without a referral.

Please be familiar with your insurance and, if you have a Managed Care Plan, please know the Preferred Providers and the requirements for your insurance.
